Laparoscopicanti-refluxsurgeryforthetreatmentofidiopathicpulmonaryfibrosis(WRAP-IPF):amulticentre,randomised,controlledphase2trial

Abnormalacidgastro-oesophagealreflux(GER)ishypothesisedtoplayaroleinprogressionofidiopathicpulmonaryfibrosis(IPF).WeaimedtodeterminewhethertreatmentofabnormalacidGERwithlaparoscopicanti-refluxsurgeryreducestherateofdiseaseprogression.

异常酸的胃食管反流（GER）被认为在特发性肺纤维化（IPF）的发展过程中起作用。我们的目标是确定腹腔镜下抗反流手术治疗不正常的胃酸是否能降低疾病恶化的速度。

TheWRAP-IPFtrialwasarandomisedcontrolledtrialoflaparoscopicanti-refluxsurgeryinpatientswithIPFandabnormalacidGERrecruitedfromsixacademiccentresintheUSA.WeenrolledpatientswithIPF,abnormalacidGER(DeMeesterscoreof≥14·7;measuredby24-hpHmonitoring)andpreservedforcedvitalcapacity(FVC).WeexcludedpatientswithaFVCbelow50%predicted,aFEV?/FVCratiooflessthan0·65,ahistoryofacuterespiratoryillnessinthepast12weeks,abody-massindexgreaterthan35,andknownseverepulmonaryhypertension.Con
